Start with setting achievable goals, create a consistent routine. Include some habits you already practice almost daily but perhaps skip on weekends, and then try to implement adding that habit to the weekend. Try a new goal such as meditating for perhaps only 2x per week, then when you accomplish such, start increasing it slowly. The Brain does not like big changes, it is not just mere will power, the brain needs to find a comfort zone and not feel threatened, this happens at a very primal subconscious level. It takes 12 weeks for the brain to accept a new habit, so be consistent.

Consider yourself successful for merely acknowledging the drop-off. HealthMonth isn’t just about the points (I know, shocking), but raising awareness to things you want to improve. So if you set goals for yourself and don’t meet them, you can reflect on why and how to change to actually achieve them. Best of luck.
